From First Visit to Retainer
Most people considering braces have a rough idea of the outcome and almost no idea of the sequence. Here is what a full course of treatment actually involves, start to finish.
The Consultation
An exam, digital X-rays, photographs and a scan of the teeth. We look at how the bite fits together rather than only how the front teeth line up, because crowding, overbite, crossbite and open bite are different problems with different solutions. You leave with the options that genuinely apply to your case and a written fee. Nothing starts that day.
Placement Day
Roughly an hour to an hour and a half. The teeth are cleaned and prepared, brackets are bonded on individually, and the first wire is placed. It does not hurt. Soreness arrives the following day and fades within three or four.
Soft food for the first week is the practical advice - pasta, eggs, soup, yogurt, anything that does not need real chewing. Over-the-counter pain relief and warm salt-water rinses cover the rest.
The Visit Rhythm
Adjustments every six to eight weeks. Each one takes fifteen to thirty minutes and involves changing the wire or adding components. There is usually a day or two of pressure afterwards. Between visits, the wire is doing the work on its own.
What Actually Shortens Treatment
Keeping every appointment, wearing elastics exactly as prescribed, and not breaking brackets. Those three things account for most of the difference between a case that finishes on estimate and one that runs six months long. Elastics in particular are the step patients skip, and skipping them stalls the bite correction entirely.
Removal and Retention
Taking the braces off takes under an hour and is painless. A retainer is fitted the same day or shortly after, and it is not optional - teeth drift for the rest of your life, and retention is what protects the result you just paid for.
What the Records Actually Show
The imaging at the first visit is not a formality. A panoramic X-ray shows every tooth present, including the ones still under the gum, and it catches the problems you cannot see in a mirror: an impacted canine sitting sideways, a permanent tooth missing entirely, a root angled into its neighbour. It also shows where the wisdom teeth are and whether they matter for your plan.
The scan replaces the old putty impressions and produces a digital model we can measure precisely. Measuring is the point. Deciding whether crowding is three millimetres or seven is the difference between aligning the teeth as they stand and needing to create space first, and that single measurement drives most of the plan and most of the fee.
Living With Braces Day to Day
Two habits do most of the work: cleaning, and not breaking anything.
Cleaning takes longer than it used to, and there is no way around it. Brackets give plaque places to sit, and plaque left around a bracket for months leaves permanent white marks on the enamel that are still there after the braces come off. Brush after every meal, use a floss threader or a water flosser nightly, and keep your regular dental cleanings on schedule with your dentist throughout treatment.
Breakage is mostly about food, and it falls into two categories: hard and sticky. Ice, nuts, hard candy, popcorn kernels, bagels, caramel, taffy. Crunchy raw vegetables and apples are fine cut into pieces. A broken bracket is not an emergency, but each one costs an unscheduled trip and a little momentum.
Elastics, and Why They Matter More Than You Think
If your plan involves bite correction, at some point you will be given elastics to hook between the upper and lower braces. They are the instruction patients quietly skip, and skipping them stalls bite correction completely - the wires keep straightening teeth while the bite relationship stays exactly where it started.
Full-time wear means full-time, minus eating and brushing. Patients who wear them as prescribed finish on estimate. Patients who wear them at night only add months, and they are usually surprised to hear the delay was avoidable.
Sports and Music
Braces and contact sports coexist fine with an orthodontic mouthguard - the type designed to fit over brackets, not the boil-and-bite kind, which moulds around the appliance and can pull on it. Wear it for practices as well as games.
Brass and wind players adapt within a few weeks. Mention it at the consultation so the start date can be planned around a concert or an audition rather than landing on top of one.
What Retention Actually Involves
A retainer is fitted the same day the braces come off or very shortly after, because teeth are at their most mobile in the first weeks. Most patients get a clear retainer, a fixed wire bonded behind the lower front teeth, or both.
Full-time wear at first, then nightly, then nightly indefinitely. Clear retainers wear out every few years and need replacing promptly - far cheaper than a second course of treatment. Fixed wires can debond quietly at one end, which is exactly what retention check-ups are for. Everyone who tells you their teeth moved back after braces is describing a retainer they stopped wearing.
What Parents Ask Most Often
Whether treatment affects school: no, beyond a few days of soreness. Whether it affects speech: not with conventional braces. Whether waiting a year would be better: sometimes it genuinely would, and we will say so, because the efficient window for comprehensive treatment is once most permanent teeth have arrived. Starting early for its own sake means a longer course of care with no better result.
What Actually Delays a Case
Missed appointments, elastics worn part-time, and repeated breakages - in that order. A wire left unchanged for four months is not working for four months, and it is the quietest way to add a season to treatment.
What does not reliably speed a case up: vibration devices, which have not held up in controlled comparisons, and light therapy, which remains unsettled. Bone remodels at the pace it remodels, and any plan that promises to beat that deserves a direct question about what it leaves uncorrected.
